RealUI's installation is very simple and fully automated.
Please read the README document included in the download.
Installation Procedure:
Delete or move your current WTF and Interface folders to a backup folder.
Copy RealUI's Interface folder to your World of Warcraft folder.
Done!
Revision Update Procedure:
Copy RealUI's Interface folder to your World of Warcraft folder.
Done!
Note 1: Make sure to overwrite all existing files and folders.
Note 2: You won't lose any of your settings when updating RealUI.
Note 3: Visit the Change Log ("http://www.wowinterface.com/downloads/info16068-RealUI.html#changelog") to find out which addons have been modified.

The primary Action Bars in RealUI are hidden by default.
They will appear under the following conditions:
You are holding down the Ctrl button
You are in combat
You have an attackable target selected
You have a focus target
You are in a group
You are in a vehicle
There are also two action bars at the sides of the screen which will appear on mouse-over.

If you're using a Russian/Chinese/Korean client, then you will need to replace some font files found in RealUI.
These font files are located under Interface\AddOns\nibRealUI\Fonts.
